School-Year Intern Service Tracking Form & Evaluation
PILI requires Interns to complete an evaluation and service tracking form to report on the types of work you are doing and the clients you are serving as well as learn more about your experience as a PILI Intern this semester. PILI uses the information from service tracking forms to help us improve our Internship Program and to inform our funders about the program’s impact.
Your responses to this evaluation are confidential and should take you about 15 minutes to complete.
Having complete and accurate information helps us secure program funding in the future. Please note that while many of these items ask for a specific number, it is fine to provide your best estimate if specific numbers are not available.
